Filing a Formal Complaint
To ensure your complaint is processed, please follow these steps:
Obtain the Official Complaint Form
Complete All Required Sections
- Provide your full legal name, address, phone number, and email.
- Clearly describe the incident, including date, time, and location.
- Include the names or descriptions of any deputies or staff members involved.
- List any witnesses and provide their contact information, if available.
- Attach any supporting documentation, such as photos, videos, or written statements.
Have the Form Notarized
- Your signature on the complaint must be notarized before submission.
- Notary services may be available at banks, certain businesses, or the courthouse.
Submit the Completed Form
- Bring the notarized form to the front desk of the Canadian County Sheriff’s Office during business hours.
- We do not accept complaint submissions by email or phone.
Retention of Copies
- Keep a copy of the completed, notarized complaint form for your records.
